I understand that the K9 Jaeger Foundation provides service dogs at no cost to qualified applicants, but I am responsible for all K9 medical expenses, food, and transportation costs. I understand that my dog is not a service dog until K9JF trainers determine the team is ready under ADA and state law. I understand that training uses cooperative, positive reinforcement methods and I will follow the training plan set by my assigned trainer. I understand that no call / no show to scheduled training sessions will result in removal from the program. I consent to a background check as part of the application process. I understand that K9JF and its trainers are not liable for any damage caused by the dog or handler during or after the program. I understand that submitting this application does not guarantee approval. All applications are reviewed by the K9JF Board of Directors. I certify that all information provided in this application is true and accurate to the best of my knowledge.
